Step 1: Register Your Student
(Please complete both steps.)

1. Download this packet or pick up a registration packet from your home school.

2. Submit the completed packet and required documentation via 1 of 2 options:

   Option 1) Submit via email:  Once the registration packet is complete, send the packet and supporting documents to your home school’s building secretary by May 1:

• If completing the packet online, we recommend filling in the fields digitally and then selecting Print - then Save to PDF. *If you just select "save" on the PDF, it may not save the information you have typed in.
• You are able to scan in necessary documents (birth certificate, proof of residency) or take pictures and attach them when emailing the registration forms. (if a form is 2 sided- please be sure to include all sides and signatures)

          Option 2) Return to building.

• After completing the forms, hand deliver them during school hours to the school your child will attend before May 1st,

STEP 2: Wait for communication from your school.

After completing Step 1, you will be contacted by your child's school in the spring. If you do not hear back by May 1, please contact your child's school to make sure you have a scheduled time for the kindergarten screening.
